Basic Overview And Facts About Peyronie’s

The vast majority of men are born with normal penises and grow up thinking nothing about the shape of their penises, confident that their sex life will be just as good as that of the next fellow. Unfortunately, nature is not always kind to us and the good things in life come mixed with many bad things.

Some men have the unpleasant surprise to find that their penis develops scar tissue and starts bending. This condition is known to health care professionals as Peyronie’s disease.

Peyronie’s disease is a condition named after Francois de la Peyronie, surgeon to Louis XIV of France, who was the first man to describe a treatment in 1743. While the cause of this disease is not well understood, its mechanism is perfectly clear.

The “tunica albuginea” is the tough layer of connective tissue that contains the sponge-like Corpora Cavernosa. For reasons unknown, scar tissue begins to form in the “tunica albuginea”, which prevents the normal expansion of the penis during erections and, in time, bends the penis to one side or the other.

The disease may be somewhat painful in the early stages of the condition. In most cases, the pain simply goes away on its own. The only effects of this disease are the curvature, which is visible when the penis is erect, and possibly a shortening of the penis in the flaccid state. It is not contagious and is in no way related to cancer or other serious conditions.

Nearly 3 percent of the entire male population is estimated to be afflicted with Peyronie’s Disease. It usually occurs between the ages of 45 and 60, though it can also occur in men as young as 18.

While diagnosing of Peyronie’s disease is quite simple, treating it is not. Most urologists have little knowledge of possible treatments and one should seek the advice of an urologist who specializes in this disease. Some treatments that have been tried are drugs containing L-arginine and sildenafil, penile exercises and surgery in severe cases.

Causes and History of the Disease

Peyronie’s disease is probably a very old condition, one that’s been around for centuries if not since mankind’s first days on this planet. Like erectile dysfunction and other intimate problems, the condition has not been openly discussed for a long time, as men tried to hide it from the world and from themselves. In the ages when people frowned upon intimate diseases and there was little understanding of illnesses and their causes, it’s easy to guess why nobody wanted to bring up this topic. The first clinical reports on this condition were made more than 250 years ago by Francois Gigot de la Peyronie, the man whose name is still linked to the condition, although the first to mention it was Cesare Aranzi in 1587. De la Peyronie was a brilliant surgeon and one of the founders of the Royal Academy of Surgery in France. His work on the penile curvature was the first and, for a long while, the only step toward the modern attempts to cure it. Unfortunately, today we are no closer to a comprehensive definition of the causes of Peyronie’s disease than Peyronie himself was in 1743.

Most doctors agree with the assumption (as yet unproven) that Peyronie’s disease is caused by microvascular trauma during intercourse. Unfortunately, since the tunica albuginea (the layer of the penis where the condition develops) is not rich in blood vessels, the healing process does not always follow the normal course. Multiple instances of intercourse trauma and poor healing lead to the formation of a plaque, which is made of fibrin, a protein that helps blood clot. This plaque prevents the normal expansion of the penis tissues during erection, thus causing the characteristic bend.

It is also believed that the diseases develops only in men who are genetically predisposed to the accumulation of fibrin within the tunica albuginea and the expansion of plaque to healthy tissue along the dorsal and ventral midline of the shaft. However, one must keep in mind that this attempt at etiology is still speculation at this moment. Unfortunately, Peyronie’s disease is considered a rare diseases and non-life-threatening, which, unfortunately, means that only a handful of professionals are actually interested in uncovering the causes and finding a cure.

The Impact Of Penile Curvature

As mentionned above, Penile Curvature or Peyronie's Disease is an abnormal bend in the penis that occurs during erection. A man with Peyronie’s Disease has hardened or calcified tissues in his penis causing restriction and preventing the penis from expanding during an erection. This results in a curvature at that point of restriction. Though it cannot physically kill you, it can actually kill your sex life rather effectively.

Symptoms of the disease include:

  • Painful erection making intercourse difficult or impossible
  • Noticeable bend or arc in the penis with erection
  • Noticeable narrowing of the diameter of the penis with erection
  • Thick band of hard tissue within the penis

The exact cause of penile curvature is unknown but many researchers believe that the plaque of Peyronie's Disease develops following trauma (hitting or bending) or vigorous sexual activity that causes localized bleeding inside the penis. Others trace it to diabetes, circulatory disorders, or a genetic link.

Many men with Peyronie's Disease do not need any treatment. The disease sometimes clears up by itself, although it may take several years. Other men, however, do need treatment.

There are several drug treatments for those suffering from Peyronie’s Disease like Vitamin E or B-complex treatment, steroid and calcium channel blockers injections, and radiation therapy but these are mainly for the reduction of pain. The most well known treatment for the disease however, involves surgery.

So if you notice a slight bend or arc or in your penis during erection, you should immediately take action before it gets worse. The condition is typically progressive and leads to impotence or inability to have intercourse.

The Straighter the Better

The penis is considered as one of the most important organs of men. Statistics show that men are very much affected by the way their penises looks and functions. Surgery has more than one answer for this disease and, if the patient is unwilling to accept the loss of penis length that always accompanies the Nesbit Procedure, then he can choose a different course of action.

This procedure entails the complete surgical removal of the lump of plaque that develops on the penis and straightening the penis in order to bring it back to the original shape. Doctors tend to advise patients to go accept this approach in cases featuring large plaques or severely bent penises. The surgeon removes the affected part of the Tunica Albuginea and replaces the plaque with a graft material of some type.

There are several types of grafts that can be used to make up for lost tissue. The first type is autograft tissue, which is tissue taken from the patient’s own body during surgery. Another problem plaguing grafts is the fact that, sooner or later, grafts undergo postoperative contraction, thus causing a shortening of the penis and scarring. Therefore, patients should weigh the pros and cons of grafts thoroughly before applying for surgery.

The last resort of many patients who suffer from severe forms of Peyronie’s disease is a penile implant. Years ago, implants were the first choice of many doctors faced with cases of men who could not achieve a proper erection because of Peyronie’s disease, but the advent of new techniques designed to cure erectile dysfunction has made their use less frequent. However, implants are still recommended to men whom erectile dysfunction drugs cannot help.

Penile implants are plastic cylinders, either solid or inflatable, which are placed inside the Corpora Cavernosa. These cylinders are biocompatible in order to avoid triggering a response of the immune system. The surgeon can either place the implants inside the penis and let them do the work of straightening the organ, or they can try to bend the penis against the plaque in order to increase the implant’s chance of successfully correcting the curvature.

Patients should be aware that surgery is an invasive procedure that is better put off until other approaches have been found ineffective. Premature or inadequate surgery has a high rate of failure or recurrence of the disease. While, it is true that patients who have undergone successful surgery are usually satisfied with the results, it only takes a simple mistake to botch an operation and with surgery there is no going back.

Treatments of Peyronie’s – More On Traction Devices

A valid option to surgery is to use a traction device to straighten the penis. Despite the controversy surrounding such novel approaches as traction devices, several clinical studies have already been conducted in order to assess the effectiveness of this technique. A Review Of Medical Studies On Peyronie's Penile Curvature

Several clinical tests have been performed in the past to with respect to conditions associated with Peyronie's disease. As mentionned above, men who took part in clinical studies have been able to reduce their curvatures as well as reduce penile retraction, a condition often times related to the disease. Let's review the nature and results of three scientific studies conducted on conditions associated with penile curvature and Peyronie's disease.

Study #1 : Can an External Penis Stretcher Reduce Peyronie's Penile Curvature ?

Results published at the 2nd European Congress of Andrology. European Academy of Andrology in Malmö, Sweden on September 19-22, 2002.

The study was performed by the Andrology Service, Ospedale SanPaolo Polo Universitario, Milano (Italy) and the Andrology Unit, Dip. Fisiopat. Clin., Università di Firenze, Firenze (Italy).

Peyronie's fibrotic lesions frequently affect the dorsal tunica albuginea and the septum of the penis leading to penile deformity and pain during erection. The aim of the study was to investigate the efficacy of mechanical penile stretching to reduce penile deformity during erection.

Ten patients (average age 57 yrs.) affected by Peyronie's disease, apparently unmodified at least for the latest 3 months and causing penile curvature during erection were trained to use a mechanical penis stretcher. None of them complained about erectile dysfunction and penile pain. Patients were subjected to daily home penis stretcher application for a minimum of 4 hours per day for 3 to 6 months. At the end of the treatment results indicated an increase in penile length and a decrease in curvature with all patients. The treatment was well tolerated (no severe complication and no drop out occurred). These results suggest a promising use of a penis stretcher in selected Peyronie's patients affected by penile curvature without erectile dysfunction or calcific plaques.

Study #2 : Treatment with penile retraction in evolutive Peyronie's disease with external penis stretching

Results published at the 5th Congress of the European Society for Sexual and Impotence Research (ESSIR) in Hamburg, Germany on December 1-4, 2002. The scientific study was also published in the international Journal of Impotence Research (vol. 14, suppl. 4, December2002).

The study was performed by the Andrology Service of the SanPaolo Hospital University of Milan, Italy.

One of the major complaints of Peyronie's disease is penile retraction. The aim of this study was to verify the efficacy of mechanical penile stretching in evolutive Peyronie's disease. Twenty-two men (age: 18-78 years) suffering from Peyronie's disease at different stages of penile retraction were enrolled and followed-up. The penis stretcher device was used, on average, for 5 hours/day, for 3 months. No adverse side effects occurred. The data collected indicated that the stretched penis elongation increased at the end of the treatment and researchers concluded that penis stretching is effective in Peyronie's disease treatment, especially at the stage of severe penile retraction.

Study #3 : Management of penile shortening after Peyronie's disease surgery

Results published at the 11th World Congress of the International Society for Sexual and Impotence Research, held on Oct 17th-21st, 2004 in Buenos Aires, Argentina and at the 7th Congress of the European Society for Sexual Medicine, held on December 5-8, 2004 in London, UK.

The study was performed by the Urology Unit of the Gregorio Marañón Hospital in Madrid, Spain.

The objective of the study was to assess the value, in terms of increasing the length of the penis, of 8 to 12-hour daily application of a penile extender device after penile surgery for Peyronie's disease. A secondary objective was to assess the health related quality of life outcome in patients using this device.

30 men, ages 54-64 years, underwent penile surgery for Peyronie's disease. After the surgery, 15 of the 30 patients were treated with a penile extender (Andro-penis device) daily over a 4-month period. Length and girth of the penis was measured before and after surgery and after the use of the extender.

Sustained treatment for 4 months with a penile stretching device provided an increase in penile lenght from 1 to 4 cms and an increase in girth of 0,5 to 1,5 cm. A significant difference could be observed between both groups. The use of the device was generally well tolerated, only 2 patients had moderate penile pain. No other complications were recorded.

The researchers concluded that the use of the penile extender device on an 8 to 12-hour daily regimen is an effective and safe way to minimize loss of penile length in patients operated for Peyronie's disease. Its use provides a significant improvement of the conditions associated with Peyronie's disease and health related quality of life compared to the control group.
